二零一一年舍路余風采堂獎學金申請表格

	Scholarship Eligibility Requirements:   Applicant must be of Yee descent (Last name or Mother's maiden name is "Yee") One or both parents must be a Yee Family Association Member Applicant must be in full time student from Kindergarten -  College Undergraduate Must provide a copy of the latest available report card Applications must be received by July 30, 2011
	獎學金申請資格﹕ 申請人須是余姓的（母親原姓余亦可以申請參加）。 父親或母親是本堂盡足義務堂員必須辦妥入堂) 申請人須是就讀幼稚園至大專院校 申請人須有下列証件之一項﹕ (1)  幼稚園至十二年級成績表及升學証件 (2)  大學或專科學院成績表及升學証件 (3)  上述特殊技能獲獎証書及學校成績   所有申請表要在七月卅日，二零一一年前交回余風采堂。
